Our Mission:
The purpose of the Summit Hill Living at Home / Block Nurse Program is to preserve and enhance the quality of life of Summit Hill seniors in their own homes, helping them to avoid institutional placement when possible, and to enhance the community by maintaining its age diversity and by encouraging expressions of caring.
Seniors sometimes need help to remain in their homes. Neighborhoods may lose vital community members if there is no one who can help them stay. This program helps by arranging for nurses, home health aides, and volunteers, and by coordinating services.
The Living at Home Block Nurse Program works with seniors, their families, and their health care professionals so that the neighborhood can keep valued members. We advocate for seniors and assist them in identifying and making use of a wide range of resources.
Many of our services are provided free of charge. Nursing and home health aide visits may be paid for by Medicare, Medical Assistance, Alternative Care Grants, and/or private insurance plans. We have a sliding fee scale, supplemented with funds raised by the Summit Hill Living at Home Block Nurse Program.
